Direct Support Professional - Community Engagement

This is a full-time position, Monday-Friday 7:30am to 4pm. 40 hours per week.
The Community Engagement DSP will provide meaningful person-centered opportunities through planned community engagements. Plan and execute opportunities for clients to access their community through volunteer work and community exploration. Maintain/build community relationships, while encouraging client independence. Provide training in appropriate work expectations while teaching socialization and adaptive skills when necessary.
Community Engagement Direct Support Professional duties include but are not limited to:
- Conducting client orientation to their local community.
- Identifying client needs and assess skill levels.
- Helping clients solve problems and promote independence.
- Assisting clients with personal care as needed.
- Planning and supervising community integrated activities.
- Participating in implementing the clients’ Individual Service Plans (ISPs) and Behavior Plans.

- Providing clients with information on personal choice options throughout the day.
- Developing, executing, and tracking community engagements and volunteer work.
- Tracking progress in employable skills.

- Implementing behavior management techniques.
- Backfilling on OHI work floor as needed.

Summary
Provide training and supervision to direct service staff using a standardized medication administration training program. Provide nursing services to promote and restore client health. Collaborate and consult with physicians and multidisciplinary team members.
Daily and/or weekly schedules may vary and be subject to flexible work hours due to scheduled or non-scheduled work responsibilities.
Essential Functions
• Conduct Medication Administration training and Basic Health and Safety training for residential staff; use the standardized competency-based training program approved by the Department of Human Services.
• Maintain necessary documentation for staff’s training records.
• Complete client nursing assessments initially and annually; maintain a system for completion and share results with case managers.
• Promote independence by teaching clients, their family and staff about medications, self-care skills and related medical conditions.
• Protect clients and employees by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations.
• Monitor medication errors and injury reports, prepare a summary, and take corrective action.
• Conduct Diabetes and Insulin training for staff; use the standardized competency-based training program approved by the Department of Human Services.
• Complete client SELF-ADMINISTRATION OF MEDICATION ASSESSMENT initially, quarterly, and/or annually based on diagnosis.
• Complete annual HRST for necessary clients.
• Approve all electronic MARS monthly and make changes to paper MAR throughout the month as needed. Make changes to clients EMAR based on changes submitted from physicians.
• Develop a working knowledge of Medicaid, Medicare, and applicable licensing standards; attend educational workshops, review professional publications, and maintain professional qualifications.

• Cook nutritious and tasty meals according to the menus.
• Purchase food and store according to sanitation regulations.
• Manage purchasing to stay within specified food and household budgets.
• Complete a weekly inventory of food stocks.
• Make sure kitchen, dining area, and storage area are cleaned properly.
• Supervise and assist clients involved in programming.
• Assist clients in solving problems related to provided services.
• Learn and implement behavior intervention techniques.
• Evaluate the efficiency and effectiveness of the food service operation and adjust as necessary.
• Assist with client medical appointments as needed.
• Complete grocery budgeting for clients actively receiving snap benefits monthly.
• Administer medications to clients as necessary.

Summary

• Maintain a safe and healthy classroom environment and teach home and community safety, including evacuation procedures.
• Maintain clean facilities and vehicles, complete assigned cleaning tasks, report safety concerns
• Encourage participation and provide individualized engagement and for each client developed from their unique interests.
• Plan and implement community recreation and leisure activities.
• Attend Interdisciplinary Team meetings and other meetings as assigned.
• Complete timely and accurate documentation and records.
• Maintain a cooperative relationship with clients, parents/guardians, other agencies, co-workers, and other departments to foster productive teamwork and accomplish goals.
• Comply with and enforce licensing and regulatory requirements, accreditation standards and OH policy and procedures.
• Provide client transportation as assigned, which may include driving company cars, vans, or buses in accordance with OH’s safety procedures; teach clients how to navigate public transportation.
• Assist with medical appointments as needed.
• Maintain First Aid, CPR Certification and other required certification and training.

Required Education and Experience

Provide case management for assigned clients. Assist clients in their home and community.
Essential Functions
• Cooperate with the ISC to design and implement service plans for assigned clients.
• Help clients participate in the design of various community, residential, and vocational support services tailored to their needs, and help individuals access services specified in their plans.
• Monitor and report client progress while maintaining accurate records.
• Ensure that individual rights and responsibilities are known and met.
• Communicate regularly and cooperate with all interdisciplinary team members, other agencies and parents/guardians.
• Ensure the safety and health of clients by identifying and eliminating safety hazards, establishing and enforcing safety rules, and reporting accidents promptly.
• Comply with and enforce licensing and regulatory requirements, accreditation standards and OH policy and procedures.
• Assist in the training and orientation of new hires, including modules of the DSP curriculum and OJT competencies.
• Arrange for transportation when necessary.
• Make recommendations for the budget and capital purchases.
• Maintain QIDP status, including Continuing Education Requirements.
• Other duties as assigned.
